New physicals for the 2021-22 school year will be required this year!

Hendricks Regional Health is offering three different physical nights.

4/18 at Brownsburg Hospital between 4:00 PM- 6:00 PM

scheduling for Brownsburg Hospital will close on on Friday 4/16 at 11:59pm

5/2 at Greencastle High School between 11:00 AM- 1:15 PM

5/16 at Danville Hospital Between 2:00 PM – 3:45 PM

Attached is the link to sign up for a time slot. You can not attend physical night without scheduling it ahead of time. The cost of the physical will be $22.

Masks must be worn, and social distancing should be observed at all times. Please limit to one adult per student appointment. The complete physical 2021-22- Physical Form Combined exam will take approximately one hour. 

You will need to either bring a physical form with you to the event or print off the forms we have on final forms, if you choose to bring a copy not from Final forms remember you will still have to fill out the forms online too.
